1. Obtain an Affidavit of Compliance
   
   • Request an Affidavit of Compliance from an NHC staff member or download and print it directly from this page. (The affidavit is available below for your convenience.)

1. Have the Affidavit Completed by Your Healthcare Provider
   
   • Take the form to your healthcare provider and ask them to complete it.

1. Return the Completed Affidavit and Register at NHC
   
   • Bring the completed Affidavit of Compliance back to NHC.
   • Fill out a short registration form with your basic information.

1. Meet with the Case Manager
   
   • Once your registration form is complete, you will meet with our Case Manager to finalize the membership intake process.

Membership at Northern Hope Center is completely free of charge.
